FEES

The nomination entry fee is $50 for AMA members; $100 for non-members. Acceptable forms of payment:

• Check (made payable to "AMA Pittsburgh")
• Credit card - information can be completed on form and submitted with other entry paperwork
• Money order

The fee to attend the AMA Holiday luncheon is $50 for AMA members and $85 for non-members.
